*Job Overview:*
We are currently seeking compassionate and dedicated Certified Nursing Assistants (CNAs) or Licensed Nursing Assistants (LNAs) to join our team.
The ideal candidate will provide essential support to patients during their rehabilitation journey, assisting with daily activities and contributing to their overall well-being.
*Responsibilities:*
* *Patient Care Assistance:* Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting, to promote independence and recovery.
* *Mobility Support:* Help patients with mobility and transferring, ensuring their safety and adherence to prescribed rehabilitation plans.
* *Vital Signs Monitoring:* Take and record vital signs, reporting any abnormalities to the nursing team.
* *Collaboration:* Work closely with the nursing and rehabilitation team to implement care plans and provide consistent, patient-centered care.
* *Communication:* Maintain open communication with patients, families, and the healthcare team, reporting changes in patient condition and addressing concerns.
* *Safety Measures:* Implement safety measures to prevent accidents or injuries during patient care activities.
* *Documentation:* Complete accurate and timely documentation of care provided, including ADL assistance and patient observations.
* *Comfort Care:* Provide comfort care measures, including pain management and emotional support, to enhance the patient's rehabilitation experience.
*Qualifications:*
* *Certification/Licensure:* Active CNA or LNA certification/license in the state of NH
* *Active BLS.

* High school diploma or equivalent.
