The Responsibilities & Duties:
Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of RAP's and Triggers
Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
Participate in facility surveys made by authorized government agencies
Evaluate each resident's condition and pertinent medical data to determine needs
Develop and implement procedure with the DON to inform all assessment team members of the arrival of newly admitted resident
Contact and assist in scheduling participation by outside members of the care plan team, including the resident's representative and/or other interested family members
Develop preliminary and comprehensive assessment of the nursing needs of each resident, utilizing the forms required
Ensure that the assessment team is aware of the importance of completeness and accuracy in their assessment function and ensure awareness of penalties
The Qualifications:
Licensed RN in the state of PA
2 years RNAC experience in a long-term care setting, required
Managed care and bundled payment knowledge
PDPM and MDS experience required
Possess excellent oral and written communication skills with computer literacy, required
Extensive knowledge of state and federal regulations and state survey process, required
Ability to provide adequate training, oversight and correction action
